Do you take insurance?

  • I accept insurance, as an in-network provider, for Blue Cross/Blue Shield, Allways, and Medicare.

  • If you have a PPO coverage, please check with your insurance company.

  • You are responsible for knowing the amount of your deductible and copayment..  Please contact your insurance company before your PT session if you are not sure of the amount. 

  • All co-payments are due at the time of service. 

  • Copayments are payable by check, cash, or credit card at the time of session.

What if I don’t have insurance coverage?

  • If you do not have insurance coverage, my rate is $120 for the first session.  After that,  my fee is $100 per treatment session.

When do I pay you for co-pays or fees?

  • All fees and/or copays are due at the time of service.  I take payment by check, cash, and credit card.

What is your NPI number?

  • My NPI (National Provider Index) number is:  1417012626.  You’ll need that when calling your insurance company to ask about your coverage.

What if I need to cancel? 

  • As outlined on the intake form, 24 hour notice is required.  With less than 24 hour notice, except in cases of emergency, unexpected illness, or extreme weather, payment of $100 is required for the missed visit.  Your insurance company will not pay this fee; you are responsible to pay this.
