Do you take insurance?
I accept insurance, as an in-network provider, for Blue Cross/Blue Shield, Allways, and Medicare.
If you have a PPO coverage, please check with your insurance company.
You are responsible for knowing the amount of your deductible and copayment.. Please contact your insurance company before your PT session if you are not sure of the amount.
All co-payments are due at the time of service.
Copayments are payable by check, cash, or credit card at the time of session.
What if I don’t have insurance coverage?
If you do not have insurance coverage, my rate is $120 for the first session. After that, my fee is $100 per treatment session.
When do I pay you for co-pays or fees?
All fees and/or copays are due at the time of service. I take payment by check, cash, and credit card.
What is your NPI number?
My NPI (National Provider Index) number is: 1417012626. You’ll need that when calling your insurance company to ask about your coverage.
What if I need to cancel?
As outlined on the intake form, 24 hour notice is required. With less than 24 hour notice, except in cases of emergency, unexpected illness, or extreme weather, payment of $100 is required for the missed visit. Your insurance company will not pay this fee; you are responsible to pay this.
