Before Your First Session:

Please download, print, and fill out these two forms and bring them to your first session:

Your first session will include a thorough musculoskeletal evaluation to identify the source of your dysfunction.  We’ll develop an individualized treatment plan, based on this evaluation.  In addition to treatment during your session, you may be given exercises to help you progress between sessions.

FAQs:

What should I wear?   Bring loose shorts and a tank top if you like, but it is not necessary because  I provide gowns.

What should I bring with me?   If this is your first visit with me, please bring the Intake (download here) and Privacy Policy & Release (download here) forms, which you will find here.  

Should I bring medical records? You do not need to bring medical records, but if you have operative or radiology reports, or other medical records you think will be helpful, please bring them, as well as a list of your medications.

Should I bring anyone with me?   You are welcome to bring a friend or family member to your appointments.

Will it hurt?  Usually not.  Some treatments may cause discomfort at the time or later but  we always work within your level of comfort.

Do you take insurance?  No.

Will you contact my doctor?   Yes, if  you want a report sent to your doctor, please include their name on the intake sheet and sign the disclosure agreement contained in our Privacy policy and release form (see top of page to download) and let me know.  Your visits are strictly confidential unless you give me permission to share information.

How long are the sessions?  Sessions are 50 minutes long.

What if I need to cancel?  As outlined on the intake form, I require a minimum of 24 hour notice on cancellations.  With less than 24 hour notice, except in cases of emergency, unexpected illness, or extreme weather, payment of $100 is expected for the missed visit.
